Key Responsibilities

  • Deliver personal care services based on patient assignments and respond promptly to their needs.
  • Communicate effectively with the Case Manager regarding any challenges in task execution.
  • Utilize positive communication techniques while ensuring confidentiality and respect for patients and their families.
  • Maintain safety standards by using equipment correctly and addressing patient safety needs.
  • Provide personal care such as bathing, grooming, and changing bed linens as required.
  • Assist patients with dressing and undressing as needed.
  • Plan and prepare nutritious meals, including shopping for necessary items.
  • Support patients during mealtime as assigned.
  • Monitor and record vital signs as directed, ensuring compliance with care plans.
  • Observe and care for patients' skin to prevent any tissue breakdown.
  • Report significant changes in patients' conditions to the Case Manager.
  • Be aware of the dynamics involving caregivers and other individuals in the patient's environment.
  • Assist with mobility and exercise as instructed by healthcare professionals.
  • Facilitate normal range of motion and positioning as required.
  • Support self-administered medication reminders as permitted by state regulations.
  • Assist with light laundry tasks as assigned.
  • Perform range of motion exercises and other simple procedures as directed.
  • Provide respite care for patients and their families when on-site.
  • Ensure the cleanliness and organization of the patient's living space as assigned.
  • Adhere to documentation and care standards established by the organization.
  • Participate in quality improvement initiatives and team activities.
  • Respect the personal needs and environment of patients and their families.

Qualifications

  • Completion of state-required training.
  • A minimum of 75 hours of classroom and supervised practical training, with specific requirements for practical training.
  • At least six months of experience in providing personal care.
  • Must be at least 18 years old.
  • Ability to read, follow instructions, and document care provided.
  • Understanding of hospice philosophy and comfort in providing care to terminally ill patients.
  • Positive references from previous employers.
  • Ability to work independently with minimal supervision.
  • Comfortable with issues surrounding death and dying.
  • Maintain a calm demeanor in patients' homes.
  • Current CPR certification is required.
  • Exhibit patience, tact, and good personal hygiene.
  • Successful completion of a probationary period to ensure competency in required qualifications.
  • Strong oral and written communication skills.
  • Good organizational abilities.
