About the Role:

The Home Health RN at CHANS Home Health & Hospice | MaineHealth provides high-quality, patient-centered care to individuals in need of hospice services. As a Registered Nurse, you will be responsible for assessing patient needs, developing care plans, and implementing evidence-based interventions to improve patient outcomes.

Key Responsibilities:
  • Assess patient needs and develop individualized care plans
  • Provide skilled nursing care to patients in their homes
  • Collaborate with interdisciplinary teams to coordinate care and services
  • Monitor patient progress and adjust care plans as needed
  • Document patient information and outcomes in a timely and accurate manner
Requirements:
  • Current license to practice as a Registered Professional Nurse in the State of Maine
  • Current certification in Basic Life Support for Healthcare providers
  • Valid driver's license and reliable transportation
  • Ability to work independently and as part of a team
  • Strong communication and interpersonal skills
