About the Job

The Community Residences is seeking a skilled Support Coordinator to join our team. As a Support Coordinator, you will be responsible for providing support and coordination to individuals with developmental disabilities in a residential setting.

Responsibilities
  • Develop and implement Individual Plans of Care to meet client needs.
  • Monitor client progress and provide regular updates to supervisors.
  • Coordinate transportation to medical appointments and community outings.
  • Participate in training opportunities to enhance skills and knowledge.
Requirements
  • A high school diploma or equivalent is required; a Bachelor's degree in a related field is preferred.
  • At least two years of experience in human services is preferred.
  • A valid CT driver's license and good driving record are required.
Salary and Benefits

We offer a competitive salary of $42,000 - $52,000 per year, depending on experience.

The estimated salary range is based on national averages for similar positions in the United States.
