At Choice in Aging, we are seeking a skilled Geriatric Care Manager to join our team. As a Geriatric Care Manager, you will work with frail low-income elders enrolled in our MSSP Medi-Cal Waiver program, providing comprehensive care and support to help them maintain safe community living.

Key Responsibilities
  • Conduct initial health assessments and reassessments to identify functional and health limitations that impede independent living.
  • Develop and implement care plans to address the needs of our participants, including recommending, providing, and coordinating social and health services.
  • Collaborate with our multidisciplinary team to ensure seamless care coordination and support.
  • Screen applicants to our program using established criteria and screening forms to determine the level of care appropriate for each participant.
Requirements
  • Bachelor's Degree in Nursing (public health, health education, health administration, gerontology, or equivalent).
  • 1-2 years of clinical nursing experience, preferably with frail older adults or one year of case management experience.
  • Hold a valid current license to practice nursing in the State of California, a public health certificate issued by the State of California is a plus.
  • NCM must have the ability to communicate in English in both written and verbal forms.
  • Must have valid California Driver's license, clean DMV record and automobile insurance.
