We are seeking a compassionate and team-oriented Nurse Practitioner to join our In-Patient Palliative Care Consult program at Stamford Health Medical Group.
In this role, you will work collaboratively with physicians and other palliative care team members to deliver high-quality palliative care and related healthcare services to patients and families.
Responsibilities will include:
- Conducting new palliative care consults and following up on existing patients throughout the Stamford Hospital setting
- Providing routine palliative care teaching to patients, family members, and newly hired RNs
- Collaborating with the palliative care team to develop and implement individualized care plans
Requirements include:
- Licensure to practice in the state of Connecticut
- Master's degree in nursing
- Certification as a Nurse Practitioner
- Minimum of 5 years nursing experience, with clinical experience in palliative care, oncology, pain management, geriatrics, or intensive care
- Minimum of 2 years palliative care APRN experience
Advanced Certified Hospice and Palliative Nurse (ACHPN) certification is highly preferred, with an expectation to obtain certification within 2 years of start date.
