We are seeking a compassionate and dedicated Hospice Care Specialist to join our team at Interim HealthCare Staffing. As a Hospice Care Specialist, you will play a vital role in providing high-quality care and support to patients in their homes.
Key Responsibilities- Provide personalized care and support to patients in their homes, as outlined in their individual care plans.
- Assist with activities of daily living, including bathing, dressing, grooming, light housekeeping, meal preparation, and transportation.
- Help patients with ambulation, transferring, and range of motion exercises to maintain their mobility and independence.
- Provide medication reminders and document patient conditions, notifying supervisors of any concerns.
- Ensure a safe home environment by maintaining unobstructed pathways and preventing falls.
- Engage patients in activities that bring them joy, such as puzzles, games, reading, and hobbies.
- High school diploma or equivalent and active CNA certification in South Carolina.
- At least one year of experience as a CNA.
- Hospice experience preferred.
- CPR certification.
- Valid driver's license, auto insurance, and reliable transportation.
- Compassionate and empathetic nature, excellent communication skills, and ability to lift up to 50 pounds.
