Job Summary

As a Hospice RN Case Manager with Interim HealthCare Staffing, you will play a vital role in providing exceptional hospice care to patients and families. This is a unique opportunity to make a meaningful difference in the lives of others while upholding our high standards of nursing practice.

Key Responsibilities
  • Maintain a specified case management load, ensuring timely and effective care delivery.
  • Conduct comprehensive assessments of patients and families to determine hospice needs, emphasizing pain and symptom management.
  • Participate in on-call duties as defined by the office on-call policy, providing 24/7 support.
  • Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG), patients, and families.
  • Coordinate care among IDG members and external healthcare providers to ensure seamless delivery of services.
  • Evaluate and coordinate the need for services, ensuring alignment with the IDG plan of care.
  • Supervise and direct services provided by LPN/LVN, Hospice Aide, and Homemaker, ensuring high-quality care.
  • Facilitate consideration of alternative care levels when indicated by patient or family needs.
Requirements
  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in the state of South Carolina, with an active and good-standing license.
  • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or evidence of current clinical knowledge.
  • One (1) year of hospice care experience, with precepting or training experience preferred.
  • CPR certification and valid driver's license with auto liability insurance.
