Hospice RN Case Manager Job Summary

We are seeking a compassionate and skilled Hospice RN Case Manager to join our team at Interim HealthCare Staffing. As a Hospice RN Case Manager, you will play a vital role in providing exceptional care to patients and families in need.

Key Responsibilities:
  • Maintain a specified case management load to ensure timely and effective care delivery.
  • Conduct comprehensive assessments of patients and families to determine their unique needs and develop personalized care plans.
  • Collaborate with the Interdisciplinary Group (IDG) to ensure seamless care coordination and implementation of care plans.
  • Provide ongoing assessments and revisions to care plans as needed to ensure optimal patient outcomes.
  • Coordinate care among IDG members and other healthcare providers to ensure continuous assessment and implementation of care plans.
  • Supervise and direct the services provided by LPN/LVN, Hospice Aide, and Homemaker assigned to patients and families.
  • Evaluate and coordinate the need for services provided to patients and families in accordance with care plans.
  • Facilitate the consideration of alternative levels of care when indicated by patient and family needs.
Requirements:
  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in the state of South Carolina with an active and good-standing license.
  • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or evidence of current clinical knowledge.
  • One (1) year of hospice care experience.
  • One (1) year of precepting or training experience preferred.
  • CPR certification.
  • Able to provide proof of valid driver's license and auto liability insurance.
