We are seeking a highly skilled and compassionate Liver Pre-Transplant Coordinator to join our team at the University of Rochester. This is a critical role in the solid organ transplant program, working closely with patients, their support teams, and referring physicians to ensure seamless evaluation and listing processes.
Key Responsibilities- Establish and maintain strong relationships with patients, families, and healthcare providers to provide exceptional patient care and support.
- Coordinate and facilitate the pre-transplant evaluation process, ensuring timely and accurate completion of necessary steps.
- Collaborate with multidisciplinary teams to develop and implement individualized care plans, addressing the unique needs of each patient.
- Provide education and guidance to patients and their support teams on transplant-related topics, promoting informed decision-making and empowerment.
- Participate in quality improvement initiatives, contributing to the development and implementation of policies and procedures that enhance patient care and outcomes.
- Diploma, Associate, or Bachelor's degree from an accredited Nursing Program required.
- Three to five years of nursing experience, with a focus on transplant or critical care, preferred.
- Strong clinical skills, excellent communication and interpersonal abilities, and a commitment to patient-centered care.
- Licensure as a Registered Nurse in NYS required; Certification in Nursing specialty area preferred.
The University of Rochester offers a competitive salary range of $50.24 hourly, with opportunities for growth and professional development in a dynamic and supportive environment.
