RN Transplant Care Coordinator- Post-Kidney/Pancreas Outpatient Specialist

Responsibilities- Assess and plan care for transplant patients in collaboration with the multidisciplinary healthcare team.
- Provide patient and family education on transplant-related topics.
- Lead community involvement, committee work, and quality improvement initiatives.
- Graduate of a baccalaureate nursing program accredited by the Accreditation Commission for Education in Nursing (ACEN) or Commission on Collegiate Nursing Education (CCNE).
- Minimum 1 year of clinical RN experience, with 3 years preferred.
- Current RN license by applicable state requirements.
- Basic Cardiac Life Support (BLS) certification required.
- Specialty certification/training as required by the work area.
