Transition Home Healthcare is a leading home health agency serving Houston, TX and surrounding areas with a passion for delivering high-quality services to patients, families, team members, and the community.

We are seeking a dedicated Physical Therapist (PT) to join our growing team in various service areas including Northeast / Northwest / Southeast / Woodlands / Clear Lake.

Immediate need exists in Northeast Houston for Full-Time & PRN positions. As a Physical Therapist with our agency, you will have the opportunity to facilitate the rehabilitation of patients with emotional, physical or mental disabilities by implementing care plans, coordinating care with the home healthcare team, educating patients and family members about health conditions, and preparing timely clinical notes.

You will also be responsible for communicating effectively with patients, caregivers, healthcare providers, office support, and other care team members. We value professionals with experience in home health settings and knowledge of home health regulations and general healthcare compliance.

We offer a competitive pay rate of approximately $95,000 - $110,000 per year, depending on experience, as well as benefits such as:
