Position Overview
The YouthBuild Case Manager plays a crucial role in delivering comprehensive support and supplementary services to participants in the YouthBuild initiative. This position is tasked with ensuring that participants have access to essential resources, guidance, and advocacy, aimed at enhancing their executive functioning and overall well-being, thereby facilitating the achievement of their educational, personal, and professional aspirations.
Key Responsibilities:
- Participant Support:
- Deliver individualized case management to YouthBuild participants, assisting them in establishing and accomplishing personal, educational, and career objectives.
- Create and maintain tailored service plans that cater to each participant's specific needs, strengths, and challenges.
- Provide emotional support, motivation, and guidance to help participants remain engaged and driven within the program.
- Assist participants in overcoming obstacles related to the criminal justice system, foster care, or other institutional challenges.
- Resource Coordination:
- Link participants with vital services, including housing support, mental health counseling, substance abuse treatment, childcare, and transportation.
- Collaborate with community partners and service providers to ensure participants can access the resources necessary for their success.
- Engage with emergency services, shelters, and other crisis resources to guarantee participants' safety and well-being.
- Monitor and follow up on participants' utilization of resources to ensure effective support for their progress.
- Assessment and Evaluation:
- Conduct initial assessments to gain insight into participants' backgrounds, needs, and goals.
- Regularly evaluate participants' progress and modify service plans as required.
- Maintain comprehensive case notes, progress reports, and other documentation in alignment with program and funding standards.
- Collaboration and Communication:
- Work in close partnership with other YouthBuild staff, including instructors, job developers, and mentors, to provide holistic support to participants.
- Participate in team meetings, case conferences, and training sessions to stay updated on program developments and best practices.
- Engage with participants' families or support networks as appropriate to cultivate a supportive environment for the participant.
- Follow-Up and Post-Program Support:
- Maintain contact with participants after program completion to offer ongoing support and guidance as they transition to employment or further education.
- Track and report on participant outcomes, including job placements, educational achievements, and long-term stability.
- Exceptional interpersonal and communication skills, with the ability to establish trust and rapport with diverse participants.
- Strong organizational abilities, capable of managing multiple cases and meeting deadlines.
- Ability to work effectively both independently and collaboratively, with minimal supervision.
- Proficient in handling sensitive and confidential information with professionalism; knowledge of trauma-informed care practices is advantageous.
- Proficiency in Microsoft Office is required; familiarity with Google Suite and Excel is highly desirable.
- Experience with Salesforce data collection software is a plus.
- Reliable work ethic characterized by integrity and accountability.
- A bachelor's degree in social work, psychology, sociology, human services, or a related field is required; a master's degree is a plus.
- A minimum of 4 years of experience, preferably in the nonprofit or social services sector, demonstrating a commitment to at-risk transitional age youth, reentry, or dual diagnosis populations.

- Hourly wage ranges from $26.89 to $34.42 (Salary Grade 18), reflecting the entire pay range based on experience and education exceeding minimum requirements.
