Job Overview

Care Medical Practice is seeking a skilled and compassionate Nurse Practitioner to join our team. As a primary care specialist, you will collaborate closely with a multi-disciplinary team to deliver high-quality, personalized care in a clinical setting.

Key Responsibilities:

  • Provide medical care for patients in a clinical setting, working in partnership with the clinical team to deliver high-quality care.
  • Assist and support clinical personnel, including clinical nurses and allied health professionals, to ensure seamless care delivery.
  • Consult with the Medical Director and Management to support clinical personnel and ensure effective care delivery.
  • Work collaboratively with a multi-disciplinary team to create individualized treatment plans for patients, promoting a patient-centric model of care.
  • Commit to the practice of value-based care, focusing on delivering care in the appropriate setting to improve patient outcomes and reduce costs.
  • Demonstrate proficiency in the use of the practice management system and electronic medical record (EMR).
  • Perform other job-related duties as assigned.

Requirements:

  • Current New York State licensure as a Nurse Practitioner.
  • Nurse Practitioner certification from ANCC or AANP.
  • 2+ years of clinical experience in primary care.
  • Current DEA licensure.
  • Experience in family medicine and internal medicine.
  • Strong communication and relationship-building skills.
  • Entrepreneurial mindset with a commitment to improving care in underserved communities.
