About the Role

Care Design New York is seeking a highly skilled and compassionate Care Manager to join our team. As a Care Manager, you will play a vital role in helping individuals with intellectual and/or developmental disabilities achieve their long-term and short-term goals.

Key Responsibilities
  • Advocacy and Support: Advocate for and with our members to ensure informed decision making, informed consent, and appropriate guardianship.
  • Life Plan Development: Schedule, lead, and collaborate with members and their interdisciplinary team to conduct meetings and assessments, ensuring the development of a comprehensive, person-centered Life Plan that reflects the person's needs and desired life goals.
  • Implementation and Monitoring: Implement, update, and monitor Life Plans, and facilitate individualized Life Plan reviews and approval processes.
  • Integration of Supports: Ensure integration of all needed and preferred supports and services, including medical, behavioral, social, habilitation, dental, psychosocial, and community-based, and facility-based long-term supports and services.
  • Ongoing Contact: Maintain ongoing contact with critical people in a member's life.
  • Documentation and Benefits: Ensure timely submission of all documentation (Life Plan, Progress notes, etc.) and assist members with maintaining benefits such as Social Security, Supplemental Security Income, Medicaid and Medicare coverage, and Food Stamps.
  • Problem Resolution: Assist members to resolve problems in living such as housing, utilities, the judicial system, and general safety.
  • Reporting: Report abuse or neglect immediately when observed or reported.
Qualifications
  • A Bachelor's degree with 2+ years of relevant experience, OR
  • A License as a Registered Nurse with 2+ years of relevant experience, which can include any employment experience and is not limited to case management/service coordination duties, OR
  • A Master's degree with 1+ year of relevant experience.
  • Strong communication skills, including verbal and written communication skills, along with strong interpersonal and organizational skills.
  • Excellent organizational, interpersonal, and verbal and written communication skills.
  • Bilingual in English and Korean.
