Job Overview

The Inpatient Care Coordinator plays a crucial role in evaluating, planning, and managing the care of patients during their hospital stay.

This position involves collaborating with physicians and healthcare professionals to create a comprehensive care plan tailored to each patient's needs. The primary objective is to ensure that patients receive the appropriate level of care throughout their hospitalization, utilizing necessary diagnostics and resources efficiently. The Inpatient Care Coordinator is also tasked with formulating a safe discharge strategy, ensuring that patients have access to required resources for a seamless transition to their next phase of care, whether it be in a skilled nursing facility, assisted living, or home care.

Key Responsibilities:
  • Conduct clinical assessments upon patient admission and monitor progress daily.
  • Ensure that the level of inpatient care is appropriate for each patient.
  • Oversee the inpatient stay to prevent service delays and address any irregularities.
  • Identify and mitigate barriers to safe patient transitions and outpatient management.
  • Coordinate all services related to care transitions, including transfers to lower levels of care.
  • Educate patients on post-discharge instructions and the importance of follow-up care.
  • Facilitate handoffs to Skilled Nursing Case Managers or Outpatient Management Programs as needed.
  • Respond promptly to patient needs and coordinate care in emergency situations.
  • Document all activities in compliance with regulatory and contractual guidelines.
  • Generate referrals for admissions and outpatient services following established guidelines.
  • Prepare correspondence for patients in accordance with regulatory standards.
  • Safeguard patient information in line with HIPAA regulations.
  • Exhibit exceptional customer service skills in all interactions.
  • Seek guidance from supervisory staff when necessary for clinical decision-making.
  • Participate in collaborative rounds with medical directors and hospital staff.
  • Perform additional duties as assigned.
The compensation for this role ranges from $45.00 to $50.00 per hour for RNs and $37.25 to $40.00 per hour for LVNs, with variations based on individual qualifications and market factors.

Additional benefits may include a sign-on bonus, discretionary awards, and a comprehensive benefits package, including 401(k) eligibility and various paid time off options.

Qualified applicants with criminal histories will be considered in accordance with applicable hiring regulations.

Qualifications:
  • Graduation from an accredited nursing program (RN or LVN).
  • Current California nursing license.
  • Experience in a hospital setting, particularly in acute care.
  • Minimum of 2 years of clinical experience in an acute care environment.
  • Proficient typing skills (40 words per minute) with accuracy.
  • Familiarity with computer systems and office software (e.g., MS Office).
  • Ability to manage confidential information responsibly.
