Key Responsibilities
  • Case Management: Develop and implement individualized treatment plans, coordinating with multidisciplinary teams to address patient needs and goals.
  • Patient Support: Provide emotional support, education, and advocacy to patients and their families, promoting overall well-being and empowerment.
  • Communication: Maintain open and effective communication with patients, families, and healthcare professionals, ensuring seamless care coordination.
  • Documentation: Accurately and thoroughly document patient progress, treatment plans, and outcomes, maintaining confidentiality and adhering to regulatory requirements.
  • Collaboration: Work closely with other healthcare professionals, community resources, and social services to ensure comprehensive care and support.
Requirements
  • Education: Enrollment in an academic Addiction Studies program and registration with CCAPP, CAADE, or CADTP (for AOD programs).
  • Experience: 1 year of experience in a Behavioral Healthcare setting (HIV programs) and DVSS Certification for DV specific staff (for all other programs).
  • Skills: Excellent communication, interpersonal, and organizational skills, with the ability to work in a fast-paced environment.
  • Certifications: Registration or certification with a DHCS approved state level counselor certification agencies (for AOD programs).
