Key Responsibilities:
  • Conduct thorough screenings and evaluations of patients with cognitive and/or activities of daily living (ADL) impairments, formulating appropriate care plans in accordance with regulatory and clinical standards.
  • Employ standardized assessments and evidence-based practices to guide clinical interventions.
  • Deliver rehabilitative, skilled, and medically necessary treatment to patients with cognitive and/or ADL challenges, adhering to regulatory and clinical guidelines.
  • Implement various rehabilitation procedures as part of the treatment plan, including ADL retraining, therapeutic activities, exercises, modalities, and the use of supportive and assistive devices.
  • Monitor patient progress and adjust treatment plans as necessary, documenting changes in compliance with regulatory and clinical standards.
  • Evaluate treatment outcomes for effectiveness, modifying services based on patient assessments and making referrals as needed.
  • Provide consultation and support to patients, families, caregivers, and other service providers regarding physical disorders.
  • Maintain effective communication with supervisors and other healthcare team members about patient progress, obstacles, and treatment strategies.
  • Prepare comprehensive discharge summaries of services rendered in accordance with regulatory and clinical requirements.
Qualifications:
Must be a licensed Occupational Therapist.
