Job Summary:

We are seeking a highly motivated and compassionate Access Counselor to join our team at Rosecrance New Life Davenport. As an Access Counselor, you will play a critical role in facilitating client assessments, case management, and admission planning services in a safe and therapeutic environment.

Key Responsibilities:
  • Conduct thorough assessments and develop individualized treatment plans
  • Provide case management and counseling services to assigned clients
  • Collaborate with multidisciplinary teams to ensure seamless care coordination
  • Develop and maintain relationships with community resources and referral sources
  • Ensure accurate and timely documentation of client records
Requirements:
  • Bachelor's degree in Psychology, Counseling, Social Work, or related field
  • tCADC/CADC, IADC, LMHC, or LISW certification required
  • Excellent written and oral communication skills
  • Computer proficiency and ability to navigate electronic healthcare records
