Key Responsibilities:

  • Provide clinical coordination of Home Health and Hospice Services, as delegated by the Manager of Clinical Operations
  • Directly support patients and their families, ensuring high-quality care delivery
  • Collaborate with multidisciplinary teams to achieve optimal patient outcomes

 

Compensation: $70,000 - $90,000 per year, commensurate with experience
