Job Summary:

The Clinical Staff Pharmacist will be responsible for providing exceptional pharmaceutical care and improving patient outcomes. This role will involve interpreting and processing patient medication orders, maintaining accurate dispensing records, and performing dose checking and medication profile review.

Key Responsibilities:
  • Interpret and process patient medication orders accurately and efficiently.
  • Maintain accurate dispensing records, including computer medication profiles, investigational drugs, and controlled substances.
  • Perform dose checking and medication profile review upon receipt of any medication request from a clinician.
  • Interpret and understand detailed requirements of drug therapy protocols and assure orders generated are consistent with protocol requirements.
  • Use knowledge of aseptic techniques and processes to prepare medications and supervise aseptic preparation of compounded sterile products.
  • Direct technicians in the preparation of medications, ensuring accuracy, efficiency, and timeliness.
  • Assure that medications are delivered to appropriate patient care areas for administration to patients in a timely manner.
  • Perform as a Pharmacist in all Satellite or Central Pharmacy areas as needed and maintain basic Pharmacy Operation standards.
Clinical and Professional Service:
  • Provide accurate drug information as necessary and upon request based on individual orders and patient conditions to optimize patient care.
  • Document drug information and other relevant clinical activities in the information system consistently with defined policy.
  • Communicate effectively with colleagues and patients to foster quality service and care; appropriately communicate unresolved issues.
  • Routinely perform patient education and counseling in a manner consistent with defined department expectations.
  • Participate in Harvey Teams and drills as assigned.
  • Actively contribute to departmental quality improvement efforts and drug utilization evaluation programs as assigned.
  • Submit medication event reports to the electronic event reporting system (EERS) and adverse drug reaction reports to Power chart.
  • Contribute to staff development programs and in-services upon request.
  • Maintain up-to-date knowledge of new therapeutics and drug delivery technology.
Research Program Support:
  • Contribute to ongoing departmental and institutional research programs and improvement efforts upon request.
  • Monitor protocol compliance for all patients by assuring appropriate protocol enrollment and dosing and scheduling accuracy.
  • Document significant protocol deviations and variances and all other significant interventions in the provision of patient care.
  • Provide and insure optimal drug distribution services for protocol patients.
  • Offer suggestions for improvements in drug delivery policies and procedures to insure optimal patient care.
  • Assist in drug, device, or technology evaluation programs as requested.
Requirements:
  • Graduate of a College of Pharmacy accredited by the American Council of Pharmacy Education and eligible for pharmacy licensure in Tennessee.
  • Hospital pharmacy practice experience or a Doctor of Pharmacy degree preferred.
