We are seeking a skilled and compassionate Registered Nurse RN Home Health Specialist to join our team at Centerpoint Medical Center. About the Role

As a Registered Nurse RN Home Health Specialist, you will be responsible for providing high-quality, patient-centered care to patients with complex medical needs. Your primary focus will be on ensuring a seamless and efficient transition for patients coming in and out of acute care hospital settings. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Key Responsibilities
  • Conduct thorough nursing assessments to identify patient needs and develop effective care plans
  • Provide education and support to patients and their families on disease management, medication adherence, and self-care techniques
  • Collaborate with healthcare providers to ensure timely and effective communication of patient information

Requirements

To be considered for this role, you must have:
