Physical Therapist Home Health Specialist

We are seeking a skilled Physical Therapist to join our team at Phoenix Home Care & Hospice. As a Physical Therapist, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.
Job Summary:The Physical Therapist will be responsible for providing physical therapy services to patients in the home setting, in accordance with the physician's plan of care and under the supervision of a licensed Physical Therapist. The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
Responsibilities:- Provides physical therapy services to patients in the home setting, in accordance with the physician's plan of care and under the supervision of a licensed Physical Therapist.
- Participates in communication and case conference activities to promote maximum continuity and quality of patient care services.
- Exhibits sound judgment and technical skills necessary to perform assigned treatment and procedures according to established agency protocols.
- Maintains compliance with the Agency Code of Ethics.
- Demonstrates ongoing compliance with agency standards, policies, procedures, state and Federal regulations, state practice acts for licensed personnel, payer requirements.
- Practices safety, environmental, and/or infection control measures.
- Performs miscellaneous job-related duties as assigned by Supervisor.
- Active license in the state of Missouri required.
- Home Health experience preferred, not required.
- Multi-disciplinary collaboration for patient care.
