Job Summary:

SYNERGY HomeCare of Yuma is seeking a compassionate and skilled Licensed Nursing Assistant to provide high-quality, individualized care for clients in their homes. As a member of our care team, you will play a vital role in ensuring the well-being and comfort of our clients, allowing them to maintain their independence and dignity while receiving the support they need.

Key Responsibilities:
  • Deliver Personal Care: Provide hands-on personal care, including bathing, toileting, grooming, and mobility assistance, to ensure clients' physical and emotional needs are met.
  • Monitor and Document: Continuously monitor clients' physical and mental health, documenting any changes or concerns, and communicate effectively with supervisors to ensure timely interventions.
  • Support Daily Living: Assist clients with daily living activities, promoting autonomy while ensuring safety, and supporting them in maintaining their independence.
  • Administer Non-Medical Support: Provide non-medical support, including medication reminders and basic exercises as prescribed, to enhance clients' overall well-being.
  • Light Housekeeping and Meal Preparation: Perform light housekeeping and prepare nutritious meals tailored to clients' preferences and dietary restrictions, ensuring a comfortable living environment.
  • Transportation and Errands: Provide transportation to appointments or accompany clients on errands, ensuring their comfort and safety during trips.
  • Build Professional Relationships: Establish and maintain professional relationships with clients and their families, providing exceptional care and support to enhance the care experience.
  • Continuing Education: Participate in ongoing training, team meetings, and workshops to stay updated on best practices in caregiving and maintain a high level of clinical knowledge.
Qualifications:
  • A valid LNA license in Arizona is required.
  • At least 1 year of caregiving experience in personal or professional settings.
  • Strong knowledge of patient safety protocols and best practices for home-based care.
  • Excellent communication skills to work effectively with clients, families, and care teams.
  • Reliable transportation and flexibility in working schedules.
  • Ability to pass a background check.
