About the Role:

Gentiva Hospice is seeking a skilled Hospice Nurse Practitioner to join our team. As a key member of our interdisciplinary group, you will be responsible for providing high-quality patient care and support to individuals and their families.

Key Responsibilities:

  • Perform Face-to-Face (F2F) encounters to gather clinical information and support hospice diagnosis and prognosis.
  • Communicate findings to the hospice physician responsible for certifying or recertifying patient eligibility.
  • Conduct physical assessments and provide clinical findings to the physician.
  • Make recommendations to the IDG based on F2F assessment findings.

Requirements:

  • Master's or higher degree in Nursing.
  • Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body.
  • Current state license as a registered nurse.
  • Current state license or certificate as an advanced practice nurse.
  • Current national certification as a Nurse Practitioner (ANCC or AANP).
  • Collaborative agreement in place.
