LPN Visit Nurse Job Summary

We are seeking a skilled and compassionate LPN Visit Nurse to join our team at SunCrest Companion Services of Smithville. As a Licensed Practical Nurse, you will provide high-quality clinical services to patients in their homes, working under the direct supervision of a Registered Nurse.

Key Responsibilities:
  • Provide direct nursing care to patients in their homes, as prescribed by physicians.
  • Record clinical and progress notes, and coordinate other patient care activities to ensure the highest quality of patient care.
  • Work collaboratively with the healthcare team to develop and implement patient care plans.
  • Document patient visits thoroughly and accurately, following regulatory and payer requirements.
  • Participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition.
Requirements:
  • Current LPN licensure in the state of practice.
  • Minimum of one year of acute care experience, with previous home care experience preferred.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR certification required.
