Key Responsibilities- Provide nursing and administrative support to clinicians and other healthcare team members
- Conduct focused/limited nursing assessments and triage patients with routine or self-limiting medical conditions
- Recognize signs and symptoms of common medical conditions and implement planned nursing interventions as delegated by clinicians
- Contribute to the development, implementation, or modification of individualized patient plans of care under the direction of clinicians
- Perform clinical tasks and procedures under the direction of clinicians, within the scope of the LPN's professional training and individual competency
- Maintain and adjust clinicians' patient schedules for efficient patient flow
- Respond to incoming messages and requests via telephone, mail, or from patients or other departments
- Facilitate and complete follow-up duties on patients' plans of care as directed by clinicians
- Assist in coordinating patient care to support patients with complex health needs
- Graduate of an accredited school of nursing
- Practical Nurse License (Washington) within 6 months of hire or Compact License: Practical Nurse within 6 months of hire
- Practical Nurse License (Oregon) within 6 months of hire
- Basic Life Support
