Job Description:

The Certified Social Worker (CSW) plays a vital role in providing exceptional patient care at SCL Health. As a key member of our team, you will utilize your clinical expertise to perform psychosocial assessments and develop treatment plans that cater to the unique needs of each patient.

You will work collaboratively with patients, their families, healthcare providers, insurers, and community resources to identify patient needs, broker services, coordinate care, and advocate for patients. Under the direct supervision of an LCSW, you will provide comprehensive intake examinations, behavioral health assessments, and determine accurate DSM-5/ICD-10 diagnoses based on clinical information.

As a CSW, you will provide behavioral health services to individuals, couples, families, and groups, using a variety of treatment approaches and tools. You will make appropriate referrals for patients needing services outside the scope of primary care, maintain complete and timely clinical records, and provide consultation to other staff members as requested.

You will also participate in staffing of cases, provide consultation and education services to individuals, agencies, institutions, and groups as assigned, and promote the mission, vision, and values of SCL Health. Additionally, you will be responsible for reviewing client files to assess dangerousness and violence evaluation of family violence referrals, and making referrals to a variety of social services and medical resources as needed.

Requirements:
  • Masters degree in Counseling or Social Work
  • Current BLS certification or ability to obtain within 3 months of hire
  • Two (2) years clinical experience in related field
Physical Requirements:

Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.

Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy.

Expected to lift and utilize full range of movement to transfer patients.

Will also bend to retrieve, lift, and carry supplies and equipment.

Typically includes items of varying weights, up to and including heavy items.

Need to walk and assist with transporting/mbulating patients and obtaining and distributing supplies and equipment.

This includes pushing/pulling gurneys and portable equipment, including heavy items.

Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc).

May be expected to stand in a stationary position for an extended period of time.
