Nursing Care Specialist

We are seeking a skilled and dedicated Licensed Practical Nurse (LPN) to join our team at Hope Family Health. As an LPN, you will play a vital role in providing comprehensive healthcare services to our diverse patient population.
Key Responsibilities:
- Patient Care and Assessment:
- Conduct thorough patient assessments, including vital signs, medical history, and current health status.
- Administer medications, immunizations, and treatments according to established protocols.
- Monitor patient progress, provide health education, and offer support and counseling as needed.
- Collaborate with healthcare providers to develop and implement individualized care plans.
- Clinic Support:
- Assist healthcare providers with examinations, procedures, and minor surgeries.
- Prepare and maintain medical equipment, ensuring its functionality and cleanliness.
- Maintain accurate and up-to-date patient records, documenting all interactions and interventions.
- Perform basic laboratory tests and specimen collection as required.
- Coordinate referrals to other healthcare professionals or community resources when necessary.
- Patient Advocacy and Education:
- Advocate for patients' rights, needs, and preferences, ensuring their voices are heard and respected.
- Educate patients and their families about health conditions, preventive care, and self-management techniques.
- Promote healthy lifestyles and disease prevention strategies within the community.
- Collaboration and Teamwork:
- Collaborate effectively with multidisciplinary teams, including physicians, nurses, and support staff.
- Participate in team meetings, case conferences, and quality improvement initiatives.
- Foster a positive and supportive work environment, promoting open communication and teamwork.

- Strong communication and interpersonal skills.

Compensation and Benefits:
