We are seeking a highly skilled Clinical Nutrition Specialist to join our team at Hope Family Health Services. The ideal candidate will have experience working with individuals with mental illness and substance use disorders.

About the Role

The Clinical Nutrition Specialist will provide comprehensive nutritional care and education to patients, including those with mental health conditions and substance use disorders. This role requires collaboration with medical staff, behavioral health staff, clinical pharmacists, and other healthcare professionals to ensure the delivery of high-quality nutritional services.

Key Responsibilities
  1. Patient Care and Counseling:
    • Conduct thorough nutritional assessments and develop personalized nutrition plans tailored to each patient's needs.
    • Provide medical nutrition therapy (MNT) for patients with various medical and mental health conditions as well as substance use disorders.
    • Offer dietary counseling and education to patients and their families to promote healthy eating habits and lifestyle changes.
    • Monitor and evaluate patient progress, adjusting nutritional plans as necessary.
  2. Team Collaboration and Communication:
    • Collaborate with healthcare providers to integrate nutrition interventions into patient care plans.
    • Participate in interdisciplinary team meetings and case conferences to discuss patient care and develop comprehensive treatment plans.
    • Serve as a resource for healthcare team members on nutrition-related issues.
    • Work closely with mental health professionals and substance use counselors to provide coordinated care.
  3. Program Development and Implementation:
    • Develop and implement nutrition programs and services tailored to the needs of the community, including programs for individuals with mental illness and substance use disorders.
    • Conduct group education sessions, workshops, and community outreach activities to promote nutrition and wellness.
    • Create educational materials and resources for patients and the community.
  4. Compliance and Documentation:
    • Maintain accurate and up-to-date patient records, including assessments, nutritional plans, and progress notes, in compliance with policies and regulations.
    • Ensure all services are provided in accordance with relevant federal, state, and local regulations and guidelines.
    • Participate in quality improvement initiatives to enhance the effectiveness of nutrition services.
  5. Professional Development:
    • Stay current with developments in the field of nutrition and dietetics through continuing education and professional development.
    • Maintain registration and licensure as required by state and national accrediting bodies.
    • Participate in professional organizations and attend relevant conferences and workshops.
Requirements
  1. Bachelor's degree or higher in Nutrition, Dietetics, or a related field from an accredited institution.
  2. Completion of an accredited dietetic internship program.
  3. Current registration as a Registered Dietitian (RD) with the Commission on Dietetic Registration (CDR).
  4. State licensure as required.
  5. Prefer at least 1-2 years of experience in clinical nutrition, preferably in a community health setting.
  6. Experience working with individuals with mental illness and substance use disorders is highly desirable.
  7. Strong interpersonal and communication skills, with the ability to work effectively in a multidisciplinary team.
  8. Knowledge of chronic disease management and evidence-based nutrition practices.
  9. Proficiency in electronic health records (EHR) and basic computer skills.
  10. Bilingual skills (Spanish or other languages) are a plus.
Compensation and Benefits

We offer a competitive salary commensurate with experience and qualifications: $60,000 - $80,000 per year. Additionally, we provide a comprehensive benefits package, including health insurance, retirement plans, paid time off, and professional development opportunities.
