Position Overview

We are seeking a Social Worker (BSW) – Volunteer Coordinator to become an integral part of our team.

In this role, you will report directly to the Executive Director or Administrator.

Your primary responsibilities will include:

  • Providing psychosocial support to patients and their families.
  • Representing and advocating for volunteer services.
  • Conducting psychosocial assessments and managing cases.
  • Delivering education and ongoing evaluation for patients and families.

Additionally, you will offer social work expertise regarding any changes in the patient's care level.


Key Responsibilities

Your duties will encompass:

  • Managing casework and service coordination for patients needing information about family dynamics, the significance of illness, cultural and spiritual perspectives on health, long-term care planning, and advanced care planning.
  • Monitoring the safety and well-being of the patient environment.
  • Supervising all volunteer activities and assessing patient and family requests for volunteer assistance.
  • Overseeing volunteer training, maintaining personnel files, and ensuring timely documentation of volunteer visits.

Qualifications

The ideal candidate will possess:

  • A Bachelor’s Degree in Social Work from a CSWE-accredited institution.
  • Licensure as mandated by the state where the hospice operates.
  • A minimum of one year of experience as a Social Worker in a healthcare or hospice environment; three to five years is preferred.

Knowledge of the needs of terminally ill patients and their families is essential, along with an understanding of hospice care and the psychosocial aspects of illness, loss, and grief. The ability to provide support during times of crisis and cope with the emotional toll of repeated loss is crucial.
