Field HCBS Care Coordinator

As a Field HCBS Care Coordinator, you'll play a vital role in ensuring that individuals receive services in a supportive, effective, efficient, timely, and cost-effective manner. This position requires a strong understanding of person-centered care and the ability to work independently with minimal supervision.
Key Responsibilities:
- Establish person-centered goal-oriented, culturally relevant, and logical steps to ensure individuals receive services.
- Coordinate care planning, case management, disease management, discharge planning, transition planning, and addressing social determinants of health.
- Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
- Communicate with stakeholders to ensure quality coordinated care and services are provided expeditiously.
- Advocate for individuals and families as needed to ensure their needs and choices are fully represented and supported by the healthcare team.
Requirements:
- Bachelor's Degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or related human services area.
- 1+ years of experience working with individuals with long-term care needs.
- 1+ years of experience working within the community health setting in a healthcare role.
- Intermediate level of computer proficiency, including Microsoft Outlook, Teams, and multiple web applications.
- Ability to travel 75% of the time for this role.
Preferred Qualifications:
- Licensed Social Worker or clinical degree.
- 1+ years of experience working with individuals receiving services on one of the home and community-based waivers in KS.
- Background in managing populations with complex medical or behavioral needs.
