The Registered Nurse Case Coordinator plays a vital role in managing the continuum of care for designated patients, ensuring effective resource utilization, quality service delivery, and adherence to medical protocols.


Key Responsibilities:
  • Conducts initial evaluations and ongoing assessments of patient conditions.
  • Records patient case details in a comprehensive database.
  • Performs monthly chart reviews and audits as necessary.
  • Contributes to monthly case discussions by sharing relevant patient information.
  • Collaborates with the Program Director to develop and refine individualized care plans.
  • Addresses patients' medical requirements by consulting with their healthcare providers, coordinating treatment plans, and advocating for their needs.
  • Enhances understanding of medical issues affecting the target demographic.
  • Assists patients in navigating entitlements, resources, and referrals for psychosocial support.
  • Engages in Quality Assurance and Utilization Review initiatives as directed.
  • Empowers patients to participate in their care planning decisions.
  • Maintains precise and timely patient records that are accessible for review and comply with all regulations; aids in data collection for reporting and funding purposes.
  • Builds collaborative relationships within and between facilities to achieve objectives; serves as a liaison among primary care providers, specialists, and patients.
  • Advocates for patient access to necessary services.
  • Adheres to State and County regulations governing nurse case management programs.
  • Participates in outreach efforts to engage the entire target population as directed.
  • Suggests program or service modifications to address community service gaps.
  • Performs additional duties as required.
