Key Responsibilities
  • Collaborate with patients, physicians, and the care team to develop and implement individualized care plans that address the unique needs of each patient.
  • Conduct thorough assessments, identify health risks, and provide education and support to patients and their families to promote healthy behaviors and disease prevention.
  • Work closely with the care team to ensure seamless transitions of care and coordinate services to meet the needs of patients.
  • Advocate for patients and their families to ensure they receive high-quality, patient-centered care that is efficient and cost-effective.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements
  • Bachelor's Degree, Associate's Degree, or diploma in Nursing from an accredited institution.
  • Current Maryland licensure or eligibility to obtain Maryland licensure.
  • Excellent customer service and communication skills.
  • Two to three years clinical experience.
  • CPR certification is required.
