Responsibilities:
Conduct in-home health and wellness assessments for health insurance plans, including H&P evaluations and medication reviews.
Perform physical exams, check vital signs, and collect urine samples as needed.
Develop and implement individualized care plans to meet the unique needs of each patient.
Collaborate with healthcare teams to ensure seamless care coordination and communication.
Requirements:
Must have a current NP license in the state of practice and a valid NPI number.
Must have CAQH credentials and be able to work independently with minimal supervision.
Must have excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
