Job Summary

LHC Group is seeking a skilled Physical Therapist to join our Home Health team in Dublin, Georgia. As a Physical Therapist, you will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. You will work closely with the physician to develop a therapy plan of care and provide interventions aimed at improving and enhancing the patient's well-being.

Responsibilities
  • Evaluate patient's functional status and physical therapy needs.
  • Assist the physician in developing the therapy plan of care.
  • Observe and report patient's response to treatment and any changes in their condition to the supervising nurse and/or the physician.
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
  • Instruct and inform participating members of the healthcare team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
Requirements
  • Current Physical Therapy licensure in the state of practice.
  • Current CPR certification required.
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
