Senior Care Coordinator

We are seeking a compassionate and skilled Senior Care Coordinator to join our team at Lighthouse Senior Living. As a key member of our care team, you will be responsible for ensuring the health, safety, and comfort of our residents.
Key Responsibilities- Assess and reassess residents to determine their suitability for our community, in accordance with state regulations and their individual needs.
- Obtain, review, and document resident medical history, physician orders, and transfer information, and provide special instructions to the Food Service Department as needed.
- Develop and implement individualized service plans for each resident, taking into account their unique needs and circumstances.
- Monitor and update resident records, unit charts, and medical charts, and audit resident medical files on a regular basis.
- Schedule appointments and arrange transportation for residents, and complete required forms and arrange for escorts as needed.
- Inventory emergency medications and ensure that the unit has adequate supplies and equipment.
- Respond to emergency situations and provide treatment, communicate with physicians and families as needed, and report any accidents or unusual incidents.
- Supervise and train care givers, and evaluate their performance and provide coaching as needed.
- Participate in the selection of new care givers and ensure that they meet our community's standards of care.
- Communicate regularly with residents' healthcare providers and families, and support a team approach to managing their health and well-being.
- Provide backup support for the Delegating Nurse, and maintain all medications, chart medication administration, and communicate with healthcare providers as needed.
- Provide care to individual residents, including medication administration, as needed, to supplement available care giving staff.
- Monitor resident treatment, medication, and diet orders, and ensure that care givers meet their health and personal care needs.
