Job Summary:

The Community Health Outreach position at Southwestern Vermont Health Care requires a skilled professional to work collaboratively with the healthcare team to support individuals recently served in the SVMC Emergency Department. This role involves developing care initiatives in partnership with team members, leveraging expertise in care coordination and case management to address social determinants of health.

Key Responsibilities:

  • Collaborate with practice and healthcare teams to provide comprehensive care
  • Outreach to individuals recently served by the SVMC Emergency Department
  • Serve as a community referral resource within the practices
  • Document all communication with individuals in Athena
  • Work at different office locations as needed

Requirements:

  • Bachelor's degree in psychology, social work, or related field preferred
  • One year of case management experience in a community, hospital, or medical/behavioral health setting preferred
  • Knowledge of local, state, and federal resources necessary
  • Experience with utilization review and discharge planning necessary
