Job Title: Geriatrician/PCP/Medical Director

We are seeking an experienced Geriatrician/PCP/Medical Director to lead our PACE program at Edenbridge Health. As a key member of our team, you will be responsible for implementing clinical care across the healthcare delivery community, clinical outcomes, and quality improvement initiatives.

About the Role

This is a unique opportunity to help lead the first PACE program in the District of Columbia, working within a supportive and innovative leadership team. You will have the chance to work autonomously, with the support of expert PACE operators, geriatricians, and project managers.

Main Responsibilities
  • Implement clinical care across the healthcare delivery community, clinical outcomes, and quality improvement initiatives
  • Design and implement the PACE program's model of care, assessment tools, and risk stratification processes
  • Serve as the program's liaison to local, regional, and national medical and PACE communities
  • Provide direct medical care as the PCP to program participants during program launch and for the program's early phase
  • Supervise all medical care and services delivered through the program
  • Evaluate and implement clinical practice guidelines based on the best available clinical evidence
  • Ensure compliance with PACE provider regulations as they pertain to medical/clinical components of care
  • Build and maintain strong relationships with physician groups, specialists, contracted providers, hospitals, and skilled nursing/post-acute rehab facilities
  • Participate in national quality improvement efforts and develop yearly QI plans, quarterly QI reports, and year-end evaluations
Requirements
  • MD or DO degree, with current licensure in the District of Columbia or ability to get licensed quickly
  • Board certified in Geriatrics (preferred), internal medicine, or family practice
  • Employment commitment through at least January 2025
  • Completion of fellowship or advanced training in geriatrics (preferred)
  • 5+ years in medical practice for the chronic care population
  • Experience in value-based payment practice preferred but not required
  • Experience working with an interdisciplinary team and performing comprehensive geriatric assessments
  • A record of achievement in forming and implementing a vision for an organization
  • Demonstrated skills in medical administration and/or previous experience as a Medical Director
  • Experience serving low-income, diverse, and marginalized communities
