Geriatrician/PCP/Medical Director

Edenbridge Health is seeking a highly skilled Medical Director to lead our PACE program at Skyland. As a key member of our team, you will be responsible for implementing clinical care across the healthcare delivery community, clinical outcomes, and quality improvement initiatives.
Main Responsibilities:
- Design and implement the PACE program's model of care, assessment tools, and risk stratification processes.
- Lead the interdisciplinary team and care planning process to ensure high-quality patient care.
- Develop and maintain strong relationships with physician groups, specialists, and contracted providers to enhance referrals and partnerships.
- Oversee the Quality Improvement (QI) program, including participation on the Quality of Care Committee, designing and conducting quality improvement projects, and evaluating program performance relative to national data.
- Participate in national quality improvement efforts and develop peer review systems for clinical staff.
Qualifications:
- MD or DO degree with current licensure in the District of Columbia or ability to get licensed quickly.
- Board certified in Geriatrics, internal medicine, or family practice.
- 5+ years in medical practice for the chronic care population.
- Experience working with an interdisciplinary team and performing comprehensive geriatric assessments.
- A record of achievement in forming and implementing a vision for an organization.
- Demonstrated skills in medical administration and/or previous experience as a Medical Director.
- Experience serving low-income, diverse, and marginalized communities.
