Job Overview

We are seeking a skilled and compassionate Community Rehabilitation Specialist to join our team at Hand in Hand Behavioral Health. The successful candidate will be responsible for developing and implementing individualized rehabilitation plans for clients within their homes and community environments.

Key Responsibilities:

  • Conduct comprehensive assessments of clients' needs, strengths, and weaknesses to formulate personalized rehabilitation plans.
  • Facilitate skill-building and therapeutic interventions in the clients' natural environment, promoting independence and community integration.
  • Collaborate with multidisciplinary teams, including mental health professionals, physicians, social workers, and family members, to ensure coordinated care.
  • Monitor and document the progress of clients, adjusting rehabilitation plans as necessary.
  • Provide education and support to clients' families, caregivers, and the wider community regarding disability awareness, inclusive practices, and the importance of rehabilitation.
  • Advocate for the rights and needs of clients within community settings, assisting in overcoming barriers to service access and inclusion.
  • Participate in regular team meetings, case conferences, and ongoing professional development activities.

Qualifications:

  • A Bachelor's degree in Social Work, Psychology, or a related field is required.
  • Excellent knowledge of rehabilitation methodologies and community resources.
  • Strong interpersonal and communication skills, with an ability to work effectively in a multidisciplinary team.
  • Demonstrated ability to handle sensitive situations with tact and diplomacy.
  • Proficient in electronic health records (EHR) systems and Microsoft Office Suite.
  • Ability to travel locally as required to meet client needs.
