Key Responsibilities:- Assess patient needs and develop personalized care plans to achieve desired outcomes.
- Collaborate with patients, families, and healthcare teams to coordinate care and services.
- Facilitate patient access to medical and specialty providers, as well as other care coordination team support.
- Provide Chronic Care Management services to eligible patients.
- Ensure effective tracking of test results, medication management, and adherence to follow-up appointments.
- Support patient self-management of disease and behavior modification interventions.
- Participate in quality improvement activities, collecting and analyzing data.
- Experience with care planning and coordination.
- Two (2) years of nursing experience.
- Excellent communication and interpersonal skills.
- Proficiency with Microsoft Office and computer skills.
- Current LPN License.
- One (1) – Two (2) years of experience in case management (preferred).
- Current RN License (preferred).
