Key Responsibilities:

  • Empower participants to make informed decisions about their independence and well-being.
  • Assist with daily tasks, such as transportation, medical appointments, and benefits applications.
  • Educate participants on basic budgeting skills and financial management.
  • Facilitate community engagement and neighborhood relations.
  • Work with participants to set and achieve goals outlined in their individual service plan.
  • Document progress and complete required paperwork in a timely and accurate manner.
  • Collaborate with community resources to enhance overall independence.

Qualifications:
