The Director of Case Management is responsible for leading the Case Management Program at Salem Hospital, ensuring the delivery of high-quality, patient-centered care. This role involves strategic planning, staff supervision, and collaboration with the multidisciplinary team to achieve timely and cost-effective outcomes.
Key Responsibilities:- Develop and implement case management policies and procedures
- Supervise and develop staff, including RNs, SWs, and non-clinical support
- Evaluate and maintain case management skills and outcomes
- Manage all policies and procedures, ensuring adherence to key model components
- Collaborate with the multidisciplinary team to assure quality, timely, and cost-efficient care
- Strong analytical and problem-solving skills
- Ability to communicate complex information in a professional and courteous manner
- Administrative skills to supervise and develop staff
- Proficiency in data analytics and hospital-specific software applications
- Minimum 7 years acute clinical experience, with a Master's Degree and current RN license in Massachusetts
- Preferred: 5+ years of progressive management experience with demonstrated goal-oriented outcomes
