Job Summary

We are seeking a highly skilled and compassionate Community Paramedicine Specialist to join our team at the University of Virginia. As a Community Paramedicine Specialist, you will play a critical role in improving health outcomes for underserved populations by connecting them with underutilized resources.

Key Responsibilities
  • Conduct patient assessments, including vital signs and physical assessments, to identify potential health risks and develop personalized care plans.
  • Provide education and guidance to patients and their families on health-related topics, including medication management, wound care, and disease prevention.
  • Collaborate with healthcare teams, including physicians, nurses, and social workers, to ensure seamless care coordination and communication.
  • Develop and maintain relationships with community resources, including non-profit organizations, government agencies, and private providers, to ensure access to necessary services and support.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements
  • Bachelor's degree in a related field, such as nursing, public health, or emergency medical services.
  • 3 years of experience in a relevant clinical environment, including emergency medical services, community health, or public health.
  • Virginia Department of Health Emergency Medical Technician Intermediate (EMT-I) or Emergency Medical Technician Paramedic (NRP/EMT-P/Paramedic) certification.
  • Valid Virginia Driver's License (VA DL) and American Heart Association (AHA) Health Care Provider BLS certification.
  • Advanced Cardiac Life Support (ACLS) certification and Community Paramedicine Certification (CP-C) within 12 months of hire.
Working Conditions

The Community Paramedicine Specialist will work in a variety of settings, including patients' homes, community clinics, and emergency departments. The position requires frequent travel, bending, and lifting, as well as exposure to fumes, malodorous smells, and infectious diseases.
