Job Overview: The Patient Pathways Coordinator at Westchester Medical Center plays a vital role in developing and implementing patient care pathways to achieve optimal outcomes. This position involves collaborating with healthcare providers to set achievable goals and providing education to patients on timely testing completion importance. Key Responsibilities include:
- Advocating for patients through the transplant process as a navigator.
- Covering continuity of patient care with external healthcare organizations.
- Managing high-risk patient care, including multiple co-morbidities or readmission risk.
- Facilitating education on transplant evaluation, cancer treatment, and post-operative care.
- Supporting disease self-management and behavior modification interventions.
- Ensuring patient compliance with testing and interventions.
- Promoting clear communication among care teams and treating clinicians.
- Communicating emergent information to clinicians.
- Evaluating clinical care, resource utilization, and new clinical tools.
Salary and Benefits:
This position offers an estimated salary of $75,000 - $90,000 per year, along with a comprehensive benefits package including medical, dental, and vision insurance, 401(k) matching, and paid time off. Education: High school diploma or equivalent required; Bachelor's degree preferred.
