We are seeking an experienced Transitions in Care Administrator to join our team at Medlink Georgia. As a key member of our program management team, you will be responsible for overseeing the implementation and management of the Transitions in Care for Justice-Involved Populations grant.

The ideal candidate will have a strong background in program management, with experience working in healthcare or social services settings. A minimum of 2 years of experience is required, preferably in a similar role. The successful candidate will also possess excellent communication and interpersonal skills, with the ability to coordinate with various stakeholders and manage program activities effectively.

Job Overview:

This is a critical role within our organization, requiring a high level of organizational skill and attention to detail. The successful candidate will have the ability to multitask, prioritize tasks, and meet deadlines while maintaining a high level of professionalism and courtesy.

Responsibilities:

  • Lead the planning, implementation, and evaluation of the Transitions in Care program.
  • Collaborate with healthcare providers, correctional facilities, community organizations, and other stakeholders to ensure seamless transitions.
  • Ensure all program activities comply with grant requirements and reporting standards.
  • Collect, analyze, and report program data to measure outcomes and inform continuous improvement.
  • Manage the program budget, ensuring efficient use of resources and adherence to financial guidelines.
  • Supervise program staff, providing guidance, support, and professional development opportunities.
  • Work alongside the Outreach Team to develop and implement outreach strategies to engage justice-involved populations and promote program services.
  • Assist in the development and implementation of policies and procedures related to the program.

Salary: $70,000 - $90,000 per year, depending on experience.

Requirements:

  • Associate's degree in public health, Social Work, Healthcare Administration, Criminal Justice, or a similarly related field.
