Overview

We're seeking a skilled Nurse Practitioner to join our team on a PRN/Part-Time basis, working flexible schedules to meet the needs of our clients and patients. About the Role:

You will conduct comprehensive adult/geriatric assessments, medical history reviews, diagnoses, treatment plans, health education, and follow-up care according to Matrix guidelines and protocols. Collaboration with physicians, case managers, social workers, family members, caregivers, and ancillary personnel is essential to ensuring seamless patient care.

Key Responsibilities:

  • Conduct thorough assessments, documenting clear and accurate information
  • Collaborate with healthcare professionals to develop effective treatment plans
  • Provide education and support to patients and families
  • Work in various settings, including home visits, skilled facilities, and telehealth

Requirements:

  • Master's Degree or equivalent experience and licensure
  • Current RN and NP licensure with prescription authority
  • Board certification in a Matrix-approved specialty
  • 1+ year of experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
