About the Role

We are seeking a highly skilled Home Health Physical Therapist to join our team in Los Lunas, New Mexico. As a key member of our healthcare team, you will play a vital role in providing high-quality care to patients in their homes.

Job Summary

The successful candidate will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment and evaluation, you will develop a treatment plan to help improve and enhance the patient's well-being.

Key Responsibilities
  • Evaluate patient functional status and physical therapy needs, consulting with physicians as necessary.
  • Provide services within the scope of practice as defined by state laws governing physical therapy, in accordance with the plan of care and in coordination with other healthcare team members.
  • Monitor and report patient response to treatment and any changes in condition to nurse supervisors and/or physicians.
  • Treat patients to relieve pain, restore function, and maintain maximum functioning using acceptable standards of practice and evidence-based techniques.
Requirements

To be successful in this role, you will need:

  • Current Physical Therapy licensure in the state of New Mexico.
  • Current CPR certification.
  • A current driver's license and vehicle insurance, as well as access to a dependable vehicle.
