About the Role

As a Clinical Care Coordinator RN at the University of New Mexico - Hospitals, you will have the opportunity to provide direct nursing care to patients and families in their homes. You will be responsible for coordinating home healthcare activities and ensuring adherence to hospital and departmental policies and procedures.

Key Responsibilities
  • Assess patient's physical, psychological, social, and environmental status to develop a tailored care plan.
  • Provide nursing care and coordinate ancillary services to achieve treatment goals; supervise ancillary nursing staff inpatient care provision.
  • Administer medications and perform treatments for assigned patients, in accordance with medical and nursing protocols.
  • Report pertinent observations and reactions regarding changes in patient status to the appropriate team member.
  • Provide follow-up to ensure problem resolution; record observations and patient care activities.
Requirements
  • A program graduate (Essential) or Bachelor's Degree (Nonessential).
  • Nationally Accredited Nursing Graduate (Essential) or Nursing Education specialization (Nonessential).
  • 1 year directly related experience (Essential).

    CPR for Healthcare/BLS Provider or Professional Rescuers within 30 days (Essential), and RN licensure in NM or by reciprocal agreement (Essential).
Working Conditions

Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods. The work environment is office-based with minimal hazard, physical risk.
