About the Role:

We are seeking a skilled and compassionate Behavioral Health Field Care Manager to join our team at UnitedHealthcare. As a key member of our care management team, you will play a critical role in supporting individuals with complex behavioral needs to achieve their health and well-being goals.

Your Key Responsibilities:

  • Serve as primary care manager for members with high behavioral needs, providing comprehensive assessments, care planning, and coordination of services to promote optimal health outcomes.
  • Develop and implement individualized care plans that address medical, behavioral, functional, cultural, socioeconomic, and social determinants of health (SDOH) needs.
  • Collaborate with internal care teams, providers, and community resources to leverage expertise and supports to meet identified needs.
  • Monitor and update care plans, incorporating feedback from members to ensure actionable steps or indicators of progress toward care plan goals.
  • Provide coaching and supportive counseling to support member self-management of care needs, lifestyle changes, and effective utilization of community and natural supports.

Requirements:

  • Must possess a Master's degree in a behavioral health-related field, such as social work, psychology, or counseling, or a Bachelor's degree with 5+ years of direct clinician experience.
  • Current, unrestricted license in a designated field, such as LCSW, LPC, or LMFT.
  • 2+ years of experience directly serving individuals with behavioral health conditions, mental health/substance use disorders, and Medicaid populations.
  • Familiarity with state-specific behavioral health Medicaid benefits and accessing care for Medicaid recipients.
